All Sims from the infant life stage to the elder life stage can gain milestones. You don’t need to do anything specific to acquire milestones; they will naturally acquire them as they go through life and experience different things.
They can be carried on older Sims’ backs in an infant carrier if The Sims 4 Growing Together is installed. Growing Together also introduces eighteen different infant quirks, which can define their personalities further. As revealed during the official Growing Together livestream, infants also develop attachment based on whether their needs are frequently addressed by their caregivers (for example, unhappy infants are more skeptical towards other Sims). Infants can cry anytime (this will cause older Sims to get the Tense Emotion). Like toddlers, infants with stinky diapers will give older Sims an uncomfortable moodlet.

Of course, not all milestones are positive! The Sims 4 Growing Together milestones mark important events in your Sim’s life, both good and bad. Things like fires, divorces, and losing your job are also milestones that Sims can experience.
